At Wellness Transformation, we streamline the process of placing imaging orders across Florida. Our team coordinates with multiple imaging facilities to ensure quick and accurate diagnostics for all patients. Most facilities offer affordable self-pay options with HSA compatibility for your convenience.
Specialist Referrals
Following evaluation, we can help facilitate specialist referrals across Florida, ensuring timely and accurate treatment. Our team guides patients through the process, making specialized care accessible and convenient.
Referrals to specialists for evaluation of orthopedic injuries, spinal pain, physical therapists, or other disciplines as deemed appropriate.
Medical forms
Short-term out of work forms (up to 3 days) for acute illness or injury recovery- requires an acute illness/injury appt and is included in the appt fee.
Temporary disability parking permits (3 months)- requires an acute injury appt with an additional $10 fee. Client must meet criteria for disability form which will be documented in the visit note.
